To Obtain Your Medical Records:

• For immediate access to your health records, please visit My Salud online or our Healow app.

• To request your records to be emailed to you or another provider, fill out our Electronic Records Request form.

• Download and complete the Health Information Release form in English or Spanish, fax or mail it to our address listed on the form, or bring it to one of our clinics.
   

Immunization Records:

• You can request your complete immunization records directly from the state immunization public portal.

• After you have submitted your request, it can take up to 30 days for us to return the records to you.

Provider to Provider Records Request:

• If you are requesting images and have access to PowerShare, please make your request through PowerShare.

• Salud requests Continuity of Care records requests to be submitted in writing. Please download and complete the Continuity of Care form  and fax it to 303-227-6426.
